One memorable scene

Talking about the movie "Five Easy Pieces", I remember one scene.
It happens in a roadside restaurant. Jack orders wheat toast, which is not on the menu. But this adamant middle-aged waitress insists they do not have toast because it is not on the menu and she should strictly stick to the rule of the restaurant.
Jack says "You make sandwiches. So you have bread, don't you? I guess you have a toaster of some kind?"
She says "I don't make the rules"
To this Jack says "Give me a chicken salad sandwich on wheat toast, which is on the menu, no lettuce, no mayonnaise, and a cup of coffee."
The waitress says sarcastically, "A chicken sal san, hold (除く) the mayo, the lettuce and a cup of coffee. Anything else?"
"Yes, Now I would like you to hold the chicken, bring me the toast, charge me for the sandwich, and you have not broken any rules."
"You want me to hold the chicken, too?!"
"Yeah, I want you to hold it between your knees."
On this she feels insulted and indignantly orders him to leave. He knocks the drinks off the table with a sweep of his arm and leaves the restaurant.
As I was young, This wit of his looked exceptionally cool.
When I went to a restaurant with my female friend for a date weeks later, I suddenly wanted to try it. I ordered to a young waitress.
"A hot ham sandwich, no butter, no mayonnaise, and a cup of coffee."
"バターもマヨネーズも抜くんですか?"
"Yes, and I want you to hold the ham, too."
"ハムもですか?....." She said suspiciously. Then "....それならトーストがありますけど"
No fun at all. She must have thought I was crazy. But the girl, my date, who saw that movie with me, knew what I was trying at and struggled hard to hold back her laughter all the while. After we got out of the restaurant, I told her "I missed the chance of saying 'hold the ham between your knees' "
